Privacy policy
This policy explains how Barrie Peoples Pharmacy and South Barrie Clinic handle personal health information, and specifically what happens to information submitted through the contact form on this website, in accordance with Ontario's Personal Health Information Protection Act (PHIPA).
Personal health information collected in person
As a pharmacy and a medical clinic, we collect the personal health information needed to dispense medication, provide clinical care, bill OHIP and other payors, and meet legal reporting obligations. This information is recorded in our pharmacy and clinical systems and is used only by those involved in your care, billing or the operation of the practice.
What the contact form on this website collects
The "Send us a message" form on our Contact page collects your name, phone number, email address, a reason for contact you select from a list, and any message you type. This website's contact form is intended only for general, non-urgent enquiries. Please do not submit personal health information, such as details of a diagnosis, symptoms or medication, through this form. If your enquiry involves your health, call the pharmacy or the clinic instead.
How the form is transmitted and stored
Submissions from this form are sent over an encrypted HTTPS connection. This website does not store any information you enter in the contact form in a database; it is delivered directly to staff who monitor our general enquiries so that we can respond to you. We do not accept file uploads through this form. Do not use this form for anything urgent or time-sensitive: call (705) 722-4556 (pharmacy) or (705) 722-3013 (clinic) instead.
How long we keep it
Messages sent through the contact form are retained only as long as needed to respond to your enquiry and are then deleted. Personal health information collected as part of your pharmacy or clinical record is retained in accordance with the retention periods required of regulated health professionals in Ontario.
Access, correction and complaints
You have the right under PHIPA to request access to, and correction of, the personal health information we hold about you. To make a request, contact us at info@barriepeoplespharmacy.ca. If you believe your privacy rights have not been respected, you may also file a complaint with the Information and Privacy Commissioner of Ontario (IPC).
